2010-2011 Soccer Season Registration Dates: May 19, 2010 from 6 to 7 p.m. at Northlawn Park, Cottage Grove May 27, 2010 from 6 - 7 p.m. at Winnequah Park, Monona June 5, 2010 (MGSC Picnic) from 3:30 - 4:30 p.m. at Northlawn Park, Cottage Grove North Lawn Park • 900 North Parkview Street • Cottage Grove, WI 53527 REGISTRATION DEADLINE IS JUNE 12, 2010. Please note that registrations received after June 12** are subject to a $10 late registration fee and a $15 late uniform fee. ** Any player that participated in tryouts will not be assessed the late fees until June 19. TO REGISTER A PLAYER: Complete a MAYSA (Madison Area Youth Soccer Association) registration form (click here), soccer uniform order form Click here and submit along with 2 separate checks - 1 for the registration fee and 1 for the uniform fee - to the appropriate registrar. (See below for fee information.) Please insure that registration information is complete and correct, including emergency contact and physician information. Checks should be made payable to Monona Grove Soccer Club. Late registrations are not guaranteed placement on a team. No refunds will be given after July 1, 2010. Registration fees can be waived on an as needed basis by contacting Carl Pulvermacher. ------------------------------------ Grades 3 – 12 (U9-U18) divisions require a team uniform. See uniform fees below. Team t-shirts will be provided for younger ages.

Note to parents: The Monona Grove Soccer Club can always use new coaches, assistant coaches, team managers, volunteers for club positions and other volunteers. No experience needed.The time commitment can be minimal, but the benefits to the children and the club are immense. Also if you or your company would be willing to sponsor uniforms or other needed materials please let us know. If you have any questions about volunteering, please contact Carl Pulvermacher at 839-5270. Please note your willingness for volunteering or sponsorship on the registration form.
